The following requirements are mandated by the U.S. Department of State and are not negotiable:
Proof of U.S. Citizenship
A previous U.S. Passport, a birth certificate, or a Naturalization Certificate may be used.
The birth certificate must have a raised, embossed, or multi-colored seal from the city, county, or state of birth, and must show a file date reflecting that it was filed within one year of birth. Proof of citizenship must accompany the application sent to the U.S. Department of State; however it will be returned to you with the processed passport.
HOSPITAL BIRTH CERTIFICATES ARE NOT ACCEPTABLE!
Two (2) Passport Photos
The photos must meet the requirements stated on the back of Form DSP-11 application.
PHOTOS ARE AVAILABLE AT THE DISTRICT CLERK'S OFFICE FOR $10.00.
Valid identification
A valid drivers license, state issued I.D., or military ID is required to process a passport application..
Social Security Number
The Social Security number of person applying for the passport, including children.
Applicants that must appear in person
Applicants 17 years of age and under must appear in person with a parent or legal guardian.
Applications for minors under the age of 16 must be made in person by both natural parents who are named on the birth certificate.
Special requirements, effective February 1, 2008, exist for minors under 16 years of age.
