This project:

• Adapts the Texas Strategic Health Status Indicators
• Geocodes the data on a sub-county level
• Matches the indicators with HP2010
• Creates a Community Public Health Report Card
• Creates a technical guide to assist local health departments throughout Texas
• Develops protocols for sharing the data through various methods
• Assists neighboring rural county with the implementation and development of Texas Strategic Health Status Indicators as their community health profile.

This is the annual progress note, in PDF format, of M.A.P to community partners. We thank all of the communiity partners for their collaboration, providing input and sharing data for the tarrant County Health M.A.P.

In close collaboration with numerous community partners, the M.A.P. team selected the most important core health indicators for Tarrant County.

The core health indicators focus on the following health domains:

Domain I: Community Profile

Demographics and Socioeconomic data

• Population distribution (age, sex, and race/ethnicity for current year; 5 and 10 year projections)
• Poverty rate
• Per capita income
• Unemployment Rate
• Educational attainment
• All cause mortality rate
• Limited English proficiency
• Linguistic isolation

Quality of Life

• Years of Potential Life - Lost (before 75 years of age)

Health Resources

• Single parent households
• List of local public health services
• Population to health care professional ratio - primary care physicians, registered nurses, and dentists
• MUA and HPSA designation
• Ratio of Medicaid eligibles to participating primary care physicians
• Number and ratio of EMS certified individuals per 1,000 population by level of EMS certification
• List of doctors and dentist who accept medicaid
