The goal of the Tarrant County Health Monitoring and Assessment Project (M.A.P.) is to establish a community health profile as a baseline for accurate, periodic assessment of our community’s progress towards health-related objectives. This project: The M.A.P. to healthy community: M.A.P. Progress Note This is the annual progress note, in PDF format, of M.A.P to community partners. We thank all of the communiity partners for their collaboration, providing input and sharing data for the tarrant County Health M.A.P. The Indicators of Health Report Card In close collaboration with numerous community partners, the M.A.P. team selected the most important core health indicators for Tarrant County.
